"Return the item". Request submission page not working.

Good evening everyone, problem as per subject,Once the form has been filled out, the button  "confirm return" is not active.  Also, it is impossible to attach images even though I have tried supported formats. I tried attaching an image in this form as well, but it is impossible here too.Thank you very much in advance.
